Student & Student Athlete Accident Reporting / Insurance Claims

 

STUDENT & INTERCOLLEGIATE ATHLETIC ACCIDENT INSURANCE PLAN

Be aware that the Shasta College Student Health & Wellness Office is not authorized to guarantee payment of claims.  All claims are subject to review by Student Insurance and Blue Cross of California.

  The role of the Student Health & Wellness Office is limited to facilitating the completion of student accident reports
and / or insurance claims.

Student accident insurance is provided by Shasta-Tehama Joint Community College District to enrolled and registered students in the event of an accident or injury during college-sponsored and supervised activities. It may help to cover medical bills incurred by injured students and athletes. There are benefit limitations and some exclusions in the policy, so it is important to work with the Student Health & Wellness Services staff to insure optimum coverage. Students enrolled and registered in classes, at the time of injury, at Shasta College, and children of enrolled and registered students using the officially designated childcare facility on campus are covered by the student accident insurance plan.

Applying for Insurance Benefits

  • Immediately report accident to instructor, coach, trainer, Campus Security, or the Student Health & Wellness Office.
  • File a Student Accident / Injury Report with the Student Health & Wellness Office within 72 business hours.
  • Contact the Student Health & Wellness Office for an appointment to complete, if appropriate, a Student Accident Claim Form.

Please Note:

  1. This policy is secondary to any other insurance you may have.  Benefits under your primary plan must be exhausted before this program applies.
  2. DO NOT DELAY REPORTING:  Written notice of claim must be submitted within 120 days after the date of the accidental injury.
  3. Proof of loss (itemized bills) must be submitted within 90 days after services and supplies are received.
  4. The benefit period is one year starting on the date of the accidental injury.  First treatment must occur within 120 days of the date of the accidental injury.

STUDENT HEALTH INSURANCE / COLLEGE HEALTH PLAN INFORMATION

The Student Health & Wellness Office does not directly provide health insurance for students, and does not bill health insurance companies for any services provided. Students are strongly encouraged to pursue health insurance coverage as a part of meeting their healthcare needs.   The following are two companies that may provide low-cost health insurance policies designed for community college students:
