Certification Request Form -

To be completed after registration of classes each semester that you are in
attendance at Shasta College and using Veterans Educational Benefits.
Request for Military Service Credit -
(pdf)
To be completed by Chapter 30 and Chapter 1606 Veterans. A copy of the
Member 4 DD-214 or the Notice of Basic Eligibility (applies to Chapter 1606
only) will need to be submitted with
this request.
